For the first time (I think), House is shown in flashbacks, starting when House goes for his regular therapy session at the mental hospital. In this episode, we see two familiar faces from the first two episodes of the season, Alvie and the African American guy in charge of the hospital, to whom House has been going for therapy for a year.
He realises that something was wrong when House was late and didn't notice the decoration in the hallway. House retorts by saying that he was fat. However, House finally consents to describe his day. He enters the hospital through the ER to avoid Wilson, but Cuddy cathes him and gives him a patient who has lost her memory. His therapist was curious about why House agreed to take the case, but he offered no plausible explanation.
Earlier that day, House goes back to his old house, where he finds a very enthusiastic Alvie painting his house bright yellow. He got even more annoyed when he discovered that Alvie had sold most of his possessions to buy the can of paint. To reduce the destrcutive powers of his ex-roommate, House decides to take him along to the hospital. The therapist questions House's judgement, arguing that locking him in the bathroom or kicking him out would do the job as well. Turns out House needs a friend, now that Wilson has kicked him out.
In a further effort to avoid Wilson, House takes the patient on a tour of the city, hoping that she'll recognize something that will help to identify her. A donut seller finally tells her where she lives and they go there to find her distraught husband, who becomes even more distraught when he realises that his wife has no idea who he is.
Turns out Alvie was wnted by the immigration department and House sets him up with a hearing but he never went, causing House to call the officers and get him arrested. However, House presented a letter to prove that Alvie was indeed his mother's son (who is an American citizen) and Alvie wants to thank him by painting his ceiling magenta. House feels annoyed, but is secretly glad that Alvie is staying.
Later that night, after curing the patient, House goes home, to find Alvie gone and a note saying that he had gone to live with his cousin. House is alone, again. He gets himself drunk and goes out to a bar where he gets beaten up. And that brings us back to his therapy session.
"It's been a year. I've followed all your instructions but I'm still the same. I came here to find out how to be happy. And it looks like you do not have the answers to that." (hopefuly I got the speech right) House leaves the room, disgusted that his therapist couldn't give him what he wanted the most; happiness.
